Summary:

After witnessing a bizarre, traumatic incident involving a patient, Dr. Rose Cotter starts experiencing frightening occurrences that she can't explain. As an overwhelming terror begins taking over her life, Rose must confront her troubling past in order to survive and escape her horrifying new reality.

Plot Twist? 

Joel turns out to be the creature in disguise, revealing that Rose never actually left the house and is instead trapped in her own mind!

Ending: 

Rose forgives herself at that moment, but that doesn't deter the creature, which transforms into a monstrous version of Rose's mother. She sets the creature on fire and runs out of the house. With the curse seemingly over, she visits Joel in the Smile ending to apologize for pushing him out of her life

Very creepy, whole concept of the smile is freaky itself

The advertisement for this movie coming out was so creative and fun so it already made me excited for it. The twist being at the end makes it very unexpected and just wanting more of it all.
